How it Works

D4 HGV Driver Examination

Please book your Driver’s Medical on our Calender. Please bring along:

 

Your D4 form with your parts completed (including writing your name and date of birth at the bottom of each page where it is requested)
Photo ID
Your current pair of driving glasses and prescription if relevant.
Medical records of any significant medical condition that may affect you when driving. Please ask your GP surgery to print out a copy of your medical records or better still to send it to you by electronic means which you can forward to info@alphamedicol.co.uk with the title “Medical Records for Drivers Medical” before your appointment. You do not require medical records if you are fit and well and your health condition is well controlled. If after being examined the examining doctor decides medical records are required we will ask you to request them from your GP.

Taxi Driver Examination

Please check with the licensing office in your local authority:

 

that your council allows other Doctors who are not your GP to conduct your drivers medical examination, (some local authorities do not allow this)


if your council requires your medical records to be reviewed, please ask your GP surgery to print out a copy of your medical records or better still to send it to you by electronic means which you can forward to info@alphamedicol.co.uk with the title “Medical Records for Drivers Medical” before your appointment.


Please bring along:

 

the form for taxi drivers medical which is usually specific for each council. The taxi drivers medical forms and requirements are usually available to download via the council website.
Photo ID.
Your current pair of driving glasses and prescription if relevant.
